Visit the NYSED website here.
Complete form 1 online. This includes a $300 payment.
Complete form 3C. This form requests that the BACB sends your exam score to NYS. You can either fax this form to the BACB 1-720-468-4145 or upload it here.
A hard copy of your certification should arrive in the mail!
